Graves Registration
To preserve the memory of Illinois veterans, the Illinois Department of Veterans' Affairs maintains a Roll of Honor for all veterans buried in the State of Illinois. Funeral homes and entities responsible for cemeteries in Illinois regularly provide the Illinois Department of Veterans' Affairs with a completed 4WVGR form regarding veteran burials in order to keep our Roll of Honor up to date. The 4WVGRs can be filled out electronically by clicking here (4WVGR) Once completed, please print and mail it to us at:
